ElderCare Placement Advisors provides RN-led senior living placement services, helping families find appropriate adult family homes, assisted living, memory care, independent living, and other senior care options.
We take time to understand each person's medical needs, daily care requirements, preferences, location, and budget. Our team evaluates potential communities, coordinates tours, and guides families through the decision and transition process.
We also specialize in medically complex and time-sensitive placements, including hospital and rehabilitation discharges. Our nursing expertise helps us identify providers equipped to support needs such as tracheostomy care, feeding tubes, respiratory support, diabetes management, and complex behavioral or cognitive conditions.
Our placement services are provided at no cost to families.
Adult Family Home Placement
Our RN-led team helps families find adult family homes that match their loved one's medical, personal care, and social needs. We personally evaluate homes, assess caregiver capabilities, and consider everything from mobility and cognitive needs to room layouts, staffing, and daily routines. Our team also brings 25 years of experience previously owning and operating adult family homes.
Assisted Living Placement
We help families compare assisted living communities based on care needs, lifestyle, location, budget, and available services. Our team evaluates how well each community can support a resident today and as their needs change, helping families make informed decisions beyond amenities and appearances.
Independent Living Placement
For older adults who want to maintain their independence while enjoying a more convenient lifestyle, we help compare independent living communities based on location, amenities, social opportunities, budget, and future care considerations. Our goal is to help clients find a community that fits how they want to live.
Memory Care Placement
Finding appropriate care for someone living with Alzheimer's disease or another form of dementia requires careful consideration of safety, supervision, behavior, and changing care needs. Our RN-led team helps families evaluate memory care communities and adult family homes experienced in dementia care, with attention to staffing, caregiver training, and the individual's specific needs.
Hospital, Skilled Nursing & Rehab Transitions
We work with families, hospital discharge planners, social workers, and rehabilitation teams to identify appropriate care settings when returning home is no longer safe or practical. Our team helps coordinate timely transitions to adult family homes, assisted living, and memory care, and can help families evaluate and navigate skilled nursing options when appropriate.Urgent & Complex Care Placements
When care needs change unexpectedly, families may have little time to find a safe living arrangement. We prioritize urgent placement requests and use our clinical experience and local provider relationships to identify suitable options as quickly as possible, including for seniors with medically complex needs.
